Service Access

Access to our service is a collaborative process.

 

As the work we will do is generated by the individual, the referral will be a process of discussion with current care providers, support teams and, most importantly the person anticipating our support.

 

Our service has been built around supporting people who have a mental health diagnosis or associated difficulties, however, we consider both of these terms to be open-ended. As such we welcome a diversity of needs and advocate the flexibility of our service to accommodate this.

 

To begin the process of moving to Nabida Care Management we would require a written referral from an appropriate mental healthcare provider, such as a CPN, Social Supervisor or Care Manger.
